I wore clothes made by exofficio that came with permethrin, They
seemed pretty helpful but I also used deet repellent. When I wore
clothes with no permethrin the bugs would bite through them and I had
to use repellent on and under the clothes. In any case permethrin will
not keep them from biting exposed skin and the first few inches of
covered skin. I used Jungle Juice 100% deet repelent from REI but I
would probably opt for a 50-60% deet next time (100% feels worse than
the bites sometimes). I forget the rule about which to put on first
sunblock or repellent... You'll figure it out pretty quick.

Work on your thai language skills, it was pretty easy to get by in
tourist areas but it doesn't sound like you'll have that luxury.

Lots of other good tips here "Bring doubles of everything you
absolutely need to function" thats a good one, make sure the mics you
bring have a good reputation for rain and humidity. Tell production to
expect some equip losses despite your best efforts.

I took Chloroquine on a show in Cape Town South Africa but stopped
after every local I talked to about it laughed and I started getting
crazy dreams which is not listed as a side effect but went away when I
stopped taking it. That being said if I were going to a malaria hot
spot I would absolutely take it.
